It is possible for someone to submit the document for change of Residence Certificate. However, the document to confirm that it is delegated by him/her self or the head of the household.

2. Where to apply: Ward Residents Section of ward office, Citizens' Center of branch office

3. How to apply: Directly at the counter of ward office.

4. Office hours: 8:30am - 5:00pm, Monday - Friday

8:30am - 0:30pm every 2nd and 4th Saturdays

(Citizens' Centers of branch offices are not open on the 2nd and 4th Saturdays)

5. Holidays: Saturdays (excluding the 2nd and 4th Saturday of each month) Sundays, National holidays, December 29th -January 3rd

6. What to bring:

(1) A document to confirm that it is delegated by him/her self or the head of the household. (*1)

*1. A legal proxy must submit a document proving his/her qualification such as a family registry, and proxies other than a legal proxy must submit a letter of attorney.

(2) Any of the following identification documents of the person who comes to the counter.

a) License, certificate, or qualification (with photo) issued by a government office such as a Basic Resident Registration Card, passport, or driver’s license with photo

b) Two documents verifying the name of the applicant such as a Health Insurance Card, cash card, or bankbook

(3) National Health Insurance Card, Long Life Medical Care Insurance Card (those who have enrolled it)

(4) Long-Term Care Insurance Card, Elder Beneficiary Certificate (those who have it)

(5) Child Medical Care Certificate, Medical Care Certificate for Single-Parent Households (those who have it)

(6) Citizen Card / Seal Registration Certificate (those who have registered it)

[when your notification of moving out is determined, the seal registration certificate of Kawasaki City will be deleted.)

(7) In case of foreign nationals, in addition to the above, any of the following: Resident Card,Special Permanent Resident Certificate, or Alien Registration Card of all the people moving in. (If it is not possible to confirm the status of residence and period of stay through the Resident Card or Alien Registration Card, please bring also the passport or the Certificate of Status of Residence. Also, for people in possess of an Alien Registration Card who are in a special exception period of stay, please bring also the passport, the receipt for acceptance or receipt of application stating it.)
