How do I request a Family Register Related Certificate via overseas mail?

Answer

If requesting a Certified Copy of Your Family Register (Excerpt), Certified copy of Closed Family Register via overseas mail, send the following documents to the Kawasaki City Mail Request Center.

1. Request Form (While there is no set format, be sure to include the below items.)

(1) Type of Certificate:

Certificate of All Registered Matters on Family Register (Koseki Tohon),

Certificate of Individual Registered Matter on Family Register (Koseki Shohon),

Certificate of All Registered Matters to removal from the family register (Joseki Tohon),

Certificate of Individual Registered Matters to Removal from the Family Register (Joseki Shohon),

Certificate of Partial Registered Matter on Family Register (Koseki Ichibu Jiko Shomei),

Certificate of Partial Registered Matters to Removal from the Family Register (Joseki Ichibu Jiko Shomei),

Identification Card, Certified Copy of Supplementary Family Register (Koseki no Fuhyo),

Certified Copy of Supplementary to Removal from Family Register (Jofuhyo)

(2) Required Number of Copies

(3) Permanent domicile (Include Street Number and Town, Name for Head of the Family

(4) Name of Person(s) Written on Certificate

(5) Purpose of Use (Reason for Request)

(6) Address and Name of Requester, Signature and Seal, Phone number

(7) Your relation, if the requester and the person noted in the certification are different.

2. The documents verifying identity

(1) Copy of the documents verifying person itself such as Driver’s license in Japan or Passport

(2) Copy of the official documents verifying an Address for reply and Name, (Payment for utility bill or mail with a postmark)

(3) If it can be confirmed the identity and address for reply with driver’s license of overseas, a copy of the driver’s license of overseas can be used.

3. Reply Envelope (Write the return address. However, do not apply a stamp.)

4. Issue Fee of certificates or Return Postage

(1) Certificate of All Registered Matters on Family Register (Koseki Tohon),

Certificate of Individual Registered Matter on Family Register (Koseki Shohon),

Certificate of Partial Registered Matter on Family Register (Koseki Ichibu Jiko Shomei): 450 yen

(2) Certificate of All Registered Matters to removal from the family register (Joseki Tohon),

Certificate of Individual Registered Matters to Removal from the Family Register (Joseki Shohon), Certificate of Partial Registered Matters to Removal from the Family Register (Joseki Ichibu Jiko Shomei): 750 yen

(3) Certified Copy of Supplementary Family Register (Koseki no Fuhyo),

Certified Copy of Supplementary to Removal from Family Register (Jofuhyo): 300 yen

(4) Identification card: 300 yen

Regarding the issue fee of certificates and return postage, there is a way to have your family or friends living in japan send you. Please contact Kawasaki City Mail Request Center for more detail. (Tel: 044-987-6111)

*For the International Postal Money Order, it ended on Friday, March 27, 2020. please refer to "Related website" and "Japan Post Bank website (please refer "Information for the end of remittance to address in the United States" below for more detail.

6. Notes

(1) For your permanent dress, you are required to include your street number and the name of your town in addition to the name of the municipality and state/province/prefecture.

(2) If there is not specify otherwise, we will send the documents via air mail it takes time long since so that we kindly hope you understanding.

(3) For those who desire EMS (International Express Mail Service), please write “EMS Return Desired” on your request. Note that you will be responsible for paying the EMS fees and EMS envelope charges.

*About the special additional fee of EMS:

Due to the pandemic of COVID-19, a special EMS surcharge will be applied for the time being from June 1st 2021. For more information, please refer to "Notice of EMS Special Surcharge (Post Office website)" on the "Related page" section below.

(4) If the requester and the person on the certificate are different, a document that shows the relationship with the requester (such as a Certificate of All Registered Matters on Family Register (Koseki Tohon) or Certificate of Family Register etc.) may be required.
