If relocating within Kawasaki City (either moving to another ward in Kawasaki City or relocating within the same ward in Kawasaki City), you need to notify the address change.

1. Document to be submitted: Change-of-address notification (Jumin Ido Todoke)

2. Notifying period: Within 14 days after relocation

3. Submission counter: Ward Residents Section of Ward Office or Citizens' Center of Ward Branch Office of your new or former address

4. Notifier: The person himself/herself or the householder (Any notifier other than these persons must submit a document showing that he/she is delegated (*1). Even if the notifier is a family member of the person, he/she may be required the document showing that he/she is delegated (*1).

*1 The legal representative is required to present Family Register or other documents that prove his/her qualification as a legal representative. Any person other than the legal representative is required a power of attorney.

5. How to notify: Directly at the counter of ward office

6. Office hours:

Monday - Friday 8:30am - 5:00pm

2nd and 4th Saturdays 8:30am – 0:30pm (Citizens' Centers of branch offices are closed on the 2nd and 4th Saturdays.)

7. Holidays: Saturdays (excluding the 2nd and 4th Saturdays), Sundays, National holidays, and from December 29th to January 3rd (Citizens' Centers of branch offices are not open the 2nd and 4th Saturdays.)

8. Required Items:

(1) Any of the following identification documents

a) Any license, certificates, or certificate of qualification issued by a government agency such as My Number Card, Basic Resident Registration Card with a face photo attached, passport, or driver's license (with a face photo attached only)

b) Two documents that confirm your name such as your Health Insurance Card, Cash card, Bankbook, etc...

(2) My Number Card (If you forget to bring it on the day when you notify, please come back with it another day.)

(3) National Health Insurance Card or Long-Life Medical Care Insurance Card (those who enroll only)

(4) Long-Term Care Insurance Card or Elder Beneficiary Certificate (only those who have it)

(5) Child Medical Care Card or Medical Care Certificate for Single-Parent household (only those who have it)

(6) Basic Resident Registration Card (only those who have it)

(7) In case of representative, the document verifies that the person is delegated (*1)

*1 The legal representative must submit Family Register or other documents that prove his/her qualification as a legal representative. Any person other than the legal representative must submit a power of attorney.

(7) For foreign nationals, in addition to the items mentioned above, all the persons who change address need one of the following items: Residence Card, Special Permanent Resident Certificate, or Alien Registration Card. (If it is not possible to confirm the residence status and period of stay with either the Residence Card or the Alien Registration Card, please bring your passport or Certificate of Status of Residence as well. Besides, for those who have Alien Registration Card and are in a special exception period of stay, please bring also your passport, the receipt of acceptance, or the receipt of application stating such status.)

9. Note: The notification is not accepted if it's notified before relocation or if it's sent by post.

1 Please note that you may be required to take procedures such as school or child allowance due to the relocation of your address.

2 If you change your address in Kawasaki, there are no special procedures because the seal registration will automatically change your address in conjunction with your residence certificate. You will also be required to use your Kawasaki Citizen Card, which includes the functions of a seal registration certificate and seal registration certificate.
