<Direct Payment System>

When you make a document agreement containing the intention to use the Direct Payment System with a medical institution or a midwifery home, such medical institutions, etc. will charge directly to Kawasaki City.

If you did not use the system, or the amount of the bill is less than the amount of the allowance, you are able to apply for the Lump-Sum Childbirth Allowance at a counter in a ward office.

* There are some facilities that you cannot use the Direct Payment System. In case where it is difficult to pay necessary expenses for delivery, please consult with Insurance and Pension Division at ward offices or the Insurance and Pension Desk, Citizens' Center of Branch office.

<Required Items when applying at the service counter in a ward office>

National Health Insurance Card

The document agreement for the Direct Payment System (which is made with a medical institution or a midwifery home)

Receipt/statements issued by medical institutions

Seal (for ink pads, Stamp seals are not acceptable.)

Birth Certificate (The Mother and Child Health Handbook, a birth certificate that a doctor issue, etc.)

Passbook of a bank, etc., or any items that contains your bank account content

Even in case of stillbirth or miscarriage at 12 weeks old or older, the allowance is still paid. Please bring a doctor's certificate in such case.

* When you gave birth within six months after resigning your job (however, only if you had been employed continuously for a period of a year or more.), you may receive the Lump-Sum Childbirth Allowance from the health insurance that you enrolled before, so please check. However, in such case, you cannot receive the allowance from the national health insurance redundantly.

*It becomes invalid in two years after giving birth, and you cannot claim the allowance at the counter. Therefore, if needed, please apply earlier as much as possible.
