The pediatric medical expenses subsidy system(Shoni Iryohi Josei Seido) is a system that subsidizes the copayment of insured medical expenses (excluding the standard copayment for dietary treatment) for hospital visits and hospitalization for children between the ages of 0 and junior high school graduation who are covered by health insurance and reside in the city. The subsidy is available for children between the ages of 0 and junior high school graduation. In addition, subsidies for outpatient medical expenses for children from the fourth grade of elementary school to the third grade of junior high school will be issued after an income screening of the applicant (guardian) is conducted to determine whether or not partial payment is required.
1 Scope of subsidy
The subsidy covers the copayment of insured medical expenses for hospital visits and hospitalization (excluding the standard copayment for dietary treatment) for children from age 0 to the third grade of junior high school.
2 What is required to apply for the issuance of a medical care card
(1) Your child's health insurance card
(2) Documents proving income *Not required depending on [Method of Income Screening] (see below)
(3) Documents to verify the number (My number card or notification card for the applicant and his/her spouses)
(4) Identification documents
<In the case where the applicant (the person with higher income) is applying)

(5) If the application is made by a proxy, a letter of attorney from the applicant or the applicant's health insurance card or other proof that the proxy is acting as the applicant's representative

Only one document needed: My Number Card (Individual Number Card), driver's license, resident card, special permanent resident certificate, physical disability certificate, mental disability certificate, medical care booklet, passport, etc.
Two documents needed:Health insurance card, pension notebook, bankbook, etc.
[Method of Income Screening]
If your child is between the ages of 0 and 3rd grade of junior high school and has been taxed outside of Kawasaki City (i.e., resided outside of Kawasaki City as of January 1 of the relevant year), there are two methods of income screening: (1) inquire about income information using your My Number provided in the application form, or (2) submit "documents proving income" to us.
If taxation has been determined in Kawasaki City, income screening will be conducted using information on taxation in the city.
If the applicant (guardian) has moved in from another city, or if the applicant (guardian) lives alone in another city, "documents proving income" will not be required by making an income information inquiry to the other city (taxing municipality) concerned using the applicant's My Number.
However, since it takes a certain amount of time to receive a response from another city (taxing municipality) regarding an income information inquiry based on My Number, as a rule, medical care cards cannot be issued on the same day; therefore, those who wish to receive them on the same day must submit documents proving income.
Please submit "documents proving income" in case income has not been declared, income information has been referred to a different source, or there is no income, as a proper response will not be obtained.
If there is a guardian whose income is higher than the applicant's income stated on the medical card, it may be necessary to change the applicant in advance. If procedures are required, the applicant will be notified by mail.
3 How to use your medical care card
When you visit a medical institution, please present your medical care card and health insurance card at the counter of the medical institution.
In principle, there is no payment of medical expenses except for the maximum co-payment amount for 4th grade elementary school students through 3rd grade junior high school students.
*If the applicant (guardian) is exempt from city income tax, even if he/she is in the 4th grade of elementary school through the 3rd grade of junior high school, the co-payment for insured medical expenses for hospital visits (medical treatment, etc.) will also be covered.

Monday through Friday 8:30 a.m. to 5:00 p.m.
Second and fourth Saturday (only at the ward offices) 8:30 a.m. to 12:30 p.m.
