If applying for a city tax certificate and other such documents by mail, send the following documents to Management Desk, Municipal Residents Taxation Section of each tax offices/Management Section of Taxation Branch Office of the Related Page below “List of Municipal Taxation Counters”
(1) Application Form:
You can download from the Related Page below “Municipal Tax Application Form Download (Certificate)”.
Please fill in the following information. (You can apply by writing on a writing pad as well.)
<Items to Include>
a) Address, Name, Phone Number to be reached during the day
If moving outside the city, also include your former address in Kawasaki City.
For corporations, write the name of the CEO and include the CEO's stamp.
b) Purpose of Use (Example: Application for Child's Medical Care Certificate, Financial Institution, etc.)
c) Type, Year, and Number of the Required Certificates
<Precautions>
・For municipal and prefectural residence tax certificates, please be careful not to incorrectly write the “Fiscal Year”, as the 2022 fiscal year (Reiwa 4) certifies income from January to December of the prior year 2021 (Reiwa 3).
・Application is required if you wish to have your income noted on the tax exemption certificate for dependents etc.
・For fixed property tax certificates and other such documents, please fill in the type of property (land, home, depreciable assets), the lot number, and the building number.
・For light vehicle tax certificates (based on classification), please fill in the license plate number of the vehicle.
(2) Postal Money Order equivalent to a commission.
The commission is 300 yen per item. *
The following 2 items are free.
・Tax payment certificate for vehicle inspection on light vehicle tax (based on classification).
・Viewing during the Public Inspection Period. (While April) (Integrated Fixed Assets Register List, Copy of Fixed Asset Tax Rolls)
*The number of certificates is counted as below:
・For Tax Payment Certificate, Taxation Certificate, and Tax Exemption Certificate will be counted 1 per Tax Item, Fiscal Year, and Taxpayer.
・For Fixed Assets Described Items Certificate(Valuation / Public dues certificate), one item per Asset and Fiscal Year.
(For the land of Condominium owned house, there is a case where the number of lots exceed 10 lots or a case where the depreciable assets are different.)
・For the Viewing a tax (Supplemental tax) list (Integrated Fixed Assets Register List), 1 items per Taxpayer, Type of Assets (land/house), and Fiscal Year.

<Precautions>
・Postal Money Orders (fixed amount) can be purchased at the post office.
(There is an additional service charge for post office.)
・Prepare a money order for the exact amount.
・Do not write anything in the designated receiver field of the Postal Money Order.
(3) Return Envelope:
Apply a stamp and write the destination address.
(4) Documents Confirming the Destination such as Certificate
In principle, we will send to the following address by non-forwarding mail.
・Destination Address for Kawasaki City Tax Notice
・Headquarters or Branch Address of Corporation where can be verified with a request form etc.
a) Destination not required if the address above.
b) If you wish to have it sent to another address because you are moving, attach a copy of government-issued documents verifying your residence at the time you lived in Kawasaki City and your past residences up to the present.
(Example: Driver’s License, Health Insurance Card, Residence Certificate, etc.)
(5) Documents required when a person other than the taxpayer (individual) or a Family Member Residing in the Same Domicile applies:
A Letter of Attorney (Notification of Agent Appointment, Consent Form) or Documents confirming the rights on which the request is based.
And If the destination address for the certificate is that of the delegate/proxy, please enclose a copy of government-issued document (corporate certificate of all registered matters) that verifies that destination address.
