The Kawasaki Citizens' Card is a multifunctional card provided by the city to help improve the convenience of life in Kawasaki. Kawasaki Citizens' Card function is as stated below.
● Personal seal registration card
● User identification card (Allows for electronic applications and usage at administrative service terminal within Kawasaki)
1 If applying to have a Kawasaki Citizen's Card function as a personal seal registration card
Please complete procedures for registering a personal seal. (Please refer to the web page for information concerning personal seal registration.)
2 If applying to have a Kawasaki Citizens' Card function as a card that can be used at a Kawasaki administrative service terminal
【Procedures】
(1) Documents: User identification card application form (available at the counters)
(2) Counters: Ward office resident sections and resident centers in your region
(3) Application: Apply directly at the counter.
(4) Items required: Personal seal (informal/unregistered seals are accepted), Authorization letter is required for proxy, identity verification documents.
* Please bring certification of personal seal registration (blue magnetic card or non-magnetic white card) if a personal seal has already been registered. Please refer to the following information for making an application.

(6) Business hours: Monday - Friday, 8:30 - 17:00
2nd & 4th Saturday of the month, 8:30 - 12:30(Branch Offices Residents centers are not open on 2nd & 4th Saturdays.)
(7) Closed: Saturdays (except 2nd & 4th Sat.), Sundays, national holidays, December 29 - January 3(Branch Offices Residents centers are not open on 2nd & 4th Saturdays.)
(8) Note
●Cards will be issued on the same day if a driver's license, passport, Basic Resident Registration Card (with photo) or some form of identification (with photo) issued by a government office is provided.
●For those who do not have an identification (with photo) issued by a government office, or the application was made by a proxy, a referral form and a written reply will be sent to the address stated in the application form for verification purposes of the individual's intentions. When you have received the referral form and a written reply at your registered address, please fill in the necessary sections, and together with documents with your name shown, such as your health insurance card, please submit it to the counter so that we can issue you the card. (May take couple of days for issue.)
●Only the applicant can receive the card due to the requirement of password registration. (Proxy cannot receive the card.)
3 If applying to have a Kawasaki Citizen's Card function as a personal seal registration card and a card that can be used at a Kawasaki administrative service terminal, both procedures [1] and [2] are necessary.
Personal seal registration cards issued before May 11, 2007 (blue magnetic cards or non-magnetic white cards) can be exchanged for a Kawasaki Citizens’ Card (functioning as a new personal seal registration card) if a replacement application is completed and submitted with the old card at a corresponding counter.

(3) Application: Apply directly at counter
(4) Items required: Personal seal registration card, power of attorney for applications by proxy
