If you forgot your password, or if you entered a wrong password five times in a row and your password was locked, you will not be able to use Kawasaki administrative service terminal. Please apply for the re-registration of your password at the counter of the ward residents section or the citizens center. (If you entered the right password before it is locked, it will reset your error numbers to 0.)
The procedure is as follows.

(1) Document: User identification card password re-registration application form (available at the counter)
(2) Counter: Please apply at the counter of the ward residents section or the citizens center in your locality.
(3) Application: Please apply directly at the counter.
(4) Items Required: Kawasaki Citizens’ Card, personal seal (unregistered informal seals acceptable), power of attorney if your proxy applies for you.
(5) Fee: Free
(6) Business hours: 8:30-17:00, Monday - Friday
8:30-12:30 on the 2nd and 4th Saturdays(Citizens centers are not open on Saturdays)
(7) Holidays: Saturdays (except 2nd and 4th Sat), Sundays, national holidays, and December 29 - January 3(Citizens centers are not open on Saturdays)

● Password will be reissued on the same day if a Basic Resident Registration Card (with photo) or some form of identification (with photo) issued by a government office is provided.
●For those who do not have an identification (with photo) issued by a government office, or the application was made by a proxy, a referral form and a written reply will be sent to the address stated in the application form for verification purposes of the individual's intentions. When you have received the referral form and a written reply at your registered address, please fill in the necessary sections, and together with documents with your name shown, such as your health insurance card, please submit it to the counter so that we can issue you the card. (May take a couple of days for issue.)
● You have to come to the counter in person when you re-register your password. (Your proxy can only apply for you.)
