If you have applied for My Number Card and haven't received your card, you can't receive your card at the ward office in Kawasaki city if you submit the notification of moving-out.
Please apply for My Number Card at your new address.
If you cannot file a moving-out notification at the counter for any reason, such as suddenly moving out on short notice, you can file the notification by post/ correspondence delivery. In that case, you will be able to receive a moving-out certificate at your registered address.
To file a moving-out notification by post/ correspondence delivery, please state the following information on a sheet of writing paper, enclose it in an envelope along with a return envelope (with an 84-yen stamp attached and the address & name of the person who is notifying stated) and send it to the Ward Resident Section of ward office or Citizens' Center of Branch Office at your old address. (A moving-out certificate will be issued.)
Information:
1. Date when you will move out (or have moved out)
2. Old address and name of the householder
3. New address and name of the householder
4. Name, date of birth, and gender of all the family members who will move out
5.Signature or signature plus personal seal of the person notifying (Notifier or the householder), as well as current address
6. Telephone number at which the person who is notifying can be contacted during the day time
* If your old address is under the jurisdiction of a branch office, please send it to the Ward Residents Section of ward office.
Please include a copy of one of the followings for identification.
1. My Number Card, Certificate with photo issued by the public agency, such as Basic Resident Register Card, passport, or driver's license, Certificate of Qualification. (Foreigners can use their residence card or special permanent resident certificate.)
2. 2 documents that can be confirmed your name such as Health Insurance Card, Cash card, bankbook.
When you send a copy of Health Insurance Card, please cover your insurance number and Code/Number for Insured person with black paint to hide them before sending.
Also, if you are in possession of any of the following, please include:
- National Health Insurance Certificate (enrolled people only)
- Elderly Medical Insurance Certificate (recipients only)
- Long-term Care Insurance Certificate (holders only)
- Medical Care Certificate for children (holders only)
- Seal Registration Certificate (holders only)
- Kawasaki Citizens' Card (holders only)
(Note)
・When you make a request by post/ correspondence delivery , you will have to wait for some days to receive the moving-out certificate. Please allow enough time when you make a request.
・A request by telephone or fax is not accepted. Please be sure to send a letter of request to us by post.
・Please be aware that if you didn't file a moving-out notice within 14 days after you moved in your new address, you might be sentenced to a fine of under 50,000 yen due to the Basic Resident Register Act.
・For foreigners who are staying in Japan for mid-long term due to the Immigration Control and Refugee Recognition Act, please be aware that you may risk having your visa status canceled if you didn't file your moving-out notice for any proper reason.
