I would like to know about the "documents proving income" required to apply for the pediatric medical expenses subsidy system.

Answer

There are no income restrictions after September 2023, but since this program is subsidized by Kanagawa Prefecture, the income of the guardian must continue to be verified.We ask for your cooperation in securing the financial resources necessary to maintain this program.

In principle, the applicant in this program is the person with the higher income (the main earner) among the parents.

"Documents proving income" are those that certify the amount of income, number of dependents, and breakdown of deductions in the applicant's name required items for determining eligibility for benefits.

*If you have been taxed outside of Kawasaki City (i.e., resided outside of Kawasaki City as of January 1 of the relevant year), there are two methods of income screening: (1) inquire about income information using your My Number provided in the application form, or (2) submit "documents proving income" to us.

If taxation has been determined in Kawasaki City, income screening will be conducted using information on taxation in the city.

*If the applicant (guardian) has moved in from another city, or if the applicant (guardian) lives alone in another city, "documents proving income" will not be required by making an income information inquiry to the other city (taxing municipality) concerned using the applicant's My Number.

*However, since it takes a certain amount of time to receive a response from another city (taxing municipality) regarding an income information inquiry based on My Number, as a rule, medical care cards cannot be issued on the same day; therefore, those who wish to receive them on the same day must submit documents proving income.

*Please submit "documents proving income" in case income has not been declared, income information has been referred to a different source, or there is no income, as a proper response will not be obtained.

1 "Documents proving income" can be one of the following

a) A copy (not the original) of the notice of determination or change of special collection of inhabitant tax related to employment income, etc. (This is a document received through your employer. This is only for those whose income is only employment income. However, even if you have only employment income, if you are using both special collection and ordinary collection, please submit documents (b) or (c) instead of (a)).

b) A copy (not the original) of the resident tax determination/tax payment notification (For those who primarily operate a business, this document is mailed to you by the municipality where you lived on January 1 of the tax year to be certified).

c) Certificate of residence taxation (issued by the municipality in which the applicant's address was located on January 1 of the tax year to be certified)

d) Certificate of tax exemption for residence tax (issued by the municipality in which the applicant's address was located on January 1 of the tax year to be certified)

Copies of documents are acceptable. Submitted documents will not be returned.

Gensen Choshuhyo (Certificate of Withholding Tax), a copy of your tax return(Kakutei Shinkokusyo), etc. will not be accepted as "documents proving income".

The name and items to be certified on the "taxation certificate" may differ depending on the issuing municipality. (The required items are a breakdown of income, a breakdown of deductions, and the number of dependents.)

If the applicant is exempt from the city income tax, please prepare a certificate that shows the city income tax rate (that the applicant is exempt from the city income tax). Please note that if you submit a certificate of child allowance or other documents that do not show the percentage of income for the civic tax, you will not be eligible for full subsidy for the copayment of the insured medical expenses.

2 Taxable year for "Documents proving income"

a) Application for issuance of a medical care card

The taxation year to be examined differs depending on the validity period of the medical care card for which you are applying. Please prepare "documents proving income" for the relevant taxation year.

As a general rule, the start date of the validity period of the medical care card is the date of moving in or September 1.

The application will be reviewed based on the income for the fiscal year 2023 (calculated based on the income from January 1 to December 31, 2022).

For third-year elementary school students and third-year junior high school students, the end of the validity period of the medical care card is March 31.
