I would like to know more about the medical expense subsidy program for single-parent families.

Answer

The Single-Parent Household Medical Subsidy Program is a subsidy system for fatherless families, motherless families, and families with caregivers to subsidy copayments of medical expenses (excluding the standard copayments of dietary treatment and living). However, there is income limit to single parents, caregivers, support obligors, etc.

1 Eligible family  

Fatherless or motherless families, or families with caregivers who have a child before the first March 31(*) after he/she turned 18 years old.

(*) For those who are with disability moderately or above and students who go to high school and so on, it can be extended until the day before their 20th birthday.

2 Scope of the subsidy     

For the copayments of medical cost for hospitalization and outpatient treatment (What are not covered by the health insurance such as health checkups and so on are not subsidized.)

3 Required Items

A. If you are receiving child support allowance and apply for it in a month other than November to December ... 1, 2, 3 and 6 below

B. If you are receiving child support allowance and apply for it in a month other than November to December ... 1, 2, 3 ,6 and (7) below

C. Those who have the Stop payment Notice of Child Rearing Allowance for 2021 ... 1, 2, 3 ,6 and (7) below

D. Those who do not receive child support allowance (such as those who receive survivor's pension) ... 1, 2, 4 ,5 ,6 and (7) below

      

1. Health Insurance Card (For your children who will be Eligible)

2. The documents verifying My Number. (The documents verifying number and the

documents verifying identity)

< If the person him/herself apply, the following 2 items are required>

(1) Documents verifying number【My Number Card(Individual Number Card) or Notification Card】

(2) Documents verifying identity *

<If the representative applies, the following 3 items are required >

(1) Documents verifying applicant's Number【My Number Card(Individual Number Card) or Notification Card】

(2) A letter of attorney from the applicant or the applicant's health insurance card, etc., which can be certified as the principal's representative.

(3) Identity verification document of representative*

*Documents for identification:

・The following items can be confirmed with only one item:

My Number Card (Individual Number Card), Driver’s license, Driver's experience certificate, Resident card, Special Permanent Resident Card, Certification of physical disability/mental disability, intellectual disability and Passport.

・The following items can be confirmed with two items:

Health Insurance Card, Child Support Allowance Certificate, National Pension Book, Bank book,

* Not only the applicant, but also a relative living together, such as a grandparent, and the spouse of the caregiver are required to fill in their My Number. Please be sure to check beforehand.

3. Child Support Allowance Certification or Stop Payment Notice of Child Support Allowance

4. Certified of Family Register of applicant and children.

5. Statement of Single-Parent Household (only for those who need it)

6. Consent form for income information reference (only for those who need it)

* Only if you have a relative living with you, please fill out the form with the person him/herself signature and bring it in advance. (Children do not need to fill it out.)

7. Documents certifying income (only for those who need it)

【About inquiry of income information by My Number】

・If the applicant or a relative living with the applicant lived outside of the city of Kawasaki as of January 1 of the relevant fiscal year, such as when the applicant moved in from another city, by filling in My Number in the application form and agreeing to inquire about income information to the relevant other city (taxable municipality), the above 7 "Documents certifying income" will no longer be required.

・Please note that as a general rule, a medical certificate cannot be issued on the same day because it takes a certain amount of time to obtain a response from another city (taxable municipality) when inquiring income information by My Number.

・If you do not report your income, or if you have different references for income information, or if you do not have any income, you will not be able to obtain proper answers regarding income information. In such cases, please submit "Documents certifying income".

4 How to use

After a qualification screening, a medical and welfare certificate will be issued to qualified recipients.

When seeing a doctor, please present the medical and welfare certificate and the health insurance card together at the reception desk of the medical institution. No medical payment is required in principle.

5 Service counter for the application

Elderly Medical Insurance / Long-term care insurance/Medical expenses Expense Subsidy, Insurance and Pension Division of each ward office or Insurance and Pension Desk of each Citizens Centers.

6 Business Hours

Monday – Friday 8:30 am - 5:00 pm

2nd and 4th Saturdays 8:30 am - 0:30 pm
