The eligibility requirements and application method for child allowance are as follows.

[Eligibility Requirements]

Persons Eligible: People registered as residents of Kawasaki City and who are raising children not yet to graduate Junior high School (After birthday of 15 years old, Until March 31 of first.)

Children Eligible: Children living in Japan and have not yet to graduate Junior high School (After birthday of 15 years old, Until March 31 of first.)

*In principle, civil servants should inquire at their workplace, as the corresponding bureau will provide benefits.

At the ward office or branch office in your ward where you live.

*Application forms available at the counters.

[Office hours]

Monday – Friday: 8:30am to 5:00pm

The 2nd and 4th Saturdays: 8:30am to 0:30pm (Ward Offices Only)

*Holidays: The 1st, 3rd, and 5th Saturdays, Sundays, National holidays, and December 29th ~ January 3rd (Branch offices are also closed the 2nd and 4th Saturdays.)

1. Bank Deposit Book with Applicant’s Name

2. The documents verifying My Number. (The documents verifying number and the

documents verifying identity)

<When the person him/herself apply, the following 2 items are required.>

(1) The documents verifying number【My Number Card(Individual Number Card) or Notification Card】

(2) The documents verifying identity *

<When the proxy applies, the following 3 items are required>

(1)The documents verifying applicant’s number【My Number Card(Individual Number Card) or Notification Card or Copy of it】

(2) The things that can be proofed as a proxy of applicant such as a letter of attorney requested by a proxy.

(3) *The documents verifying proxy’s identity

* Documents verifying identity

・The following items can be confirmed with only one item:

My Number Card (Individual Number Card), Driver’s license, Driver's experience certificate (Valid only for those issued on or after April 1, 2012), Passport, Certification of physical disability, mental disability, intellectual disability and Residence card or Special permanent certificate.

・The following items can be confirmed with two items:

Health Insurance Card, National Pension Book, Child Support Allowance Certificate, Special Child-Rearing Allowance Certificate, Family Register Certificate, Welfare Benefits Certificate.

*The child and spouse who is living outside the city, it needs to fill in My Number, so please confirm in advance.

3. Copy of Health Insurance Card etc. (Only those who need it.)

If you enroll the Federation of National Public Service Personnel Mutual Aid Association or the Mutual Aid Association of Prefectural Government Personnel, and are not paid by your employer, you will need a copy of the health insurance card or a certificate of pension participation of the person him/herself. (Each association member cannot omit this document by My Number. The certificate of pension participation will be given to you at the counter when you apply, so please obtain it at your workplace and submit it at a later date.)

4. Other Required Documents

(1) If Living Separate from the Child, etc.

If living separate from the child, you are required to submit a “written petition” showing you can provide financial support. In addition, if the child lives outside of Kawasaki City, you are required to submit a certified copy of the child's Residence Certificate, or a Certificate of Items Stated in Resident Register (item listing relationship and child's My Number).

(2) If My Number cannot be confirmed

“Certificate of income” “Copy of the applicant’s Health Insurance Card or Admission Certificate of Pension” “Copy of the Child's Residence Certificate” are required.

The city will confirm the contents of the documents in the “Certificate of Income” “Copy of the applicant’s Health Insurance Card or Admission Certificate of Pension” “Copy of the Child's Residence Certificate” using an inquiry by the information provision network system using My Number, but if the result of the inquiry is not obtained, we may ask you to submit the documents separately. It may take long time for the contact to respond.

*The documents can be submitted by after authorization request. we cannot certify it until the documents are ready, but if the application is delayed, we cannot process it retroactively, so please apply as soon as possible.
