If your doctor or midwife has diagnosed you as pregnant, you should submit a "Pregnancy Notification" (including the "OTAZUNE" on the back) to the Community Care and Support Center or the Community Health and Social Welfare Station in your domicile.

If the representative submits the notification, the letter of attorney is required.

Moreover, you are able to receive health checkup tickets, so please come earlier for the procedure.

5. Documents to be submitted:

(1) Pregnancy Notification Form (Fill out the downloaded from Kawasaki city website and bring it to the counter. Or fill out the form at the counter.)

(2) Patient ID card of the medical institution where you were examined.

(3) Identity document etc.:

If the representative submits the notification, the letter of attorney is required in addition to the Pregnancy Notification.
