The procedure of withdraw for National Health Insurance is below. Required documents differ depends on the situation.

1. Notification term: In principle, within 14 days of the occurrence of the reason.

2. Notification counter: Civic Division of your ward office, Resident Record Desk, Civic Center of branch office.

(1) If you do not take the procedure such as moving-in or moving-out.

Insurance and Pension Division of ward office or Insurance and Pension Desk, Citizens Center of Branch office in your area.

(2) If you take the procedure such as moving-in or moving-out.

Ward Residents Division of ward office or Resident Record/Child allowance/School Desk, Citizens Center of Branch office in your area.

3. How to notify: Notify at the counter of ward office directly or apply on the internet.

* Regarding a digital applying on the internet, you cannot withdraw NHI due to the reason except for the enrollment Health Insurance of your work place or National Health Insurance Association.

4. Business hours: Mon-Fri 8:30am to 5:00pm

2nd and 4th Saturdays 8:30am to 0:30pm (only ward office)

*Digital application on the internet can be accepted 24 hours in principal.

5. Closed: Sat, Sun, National holiday, Dec 29th to Jan 3rd.

(2nd and 4th Saturday can be accepted in the morning at the ward office only.)

6. What to bring:

(1) to (4) are common

a) Insurance card of the person who withdraw National Health Insurance

b) My Number Card (Individual Number Card) of House holder and the person who withdraw NHI or Notification Card (listed name and address etc. should be much with Items Sated in Resident Register.)

c) 1 item of the documents verifying notifier (the documents verifying identity(driver’s license・passport etc.) or two documents issued by government office.)

※If a person other than a member of the same household as the person who withdraw from the family submits the notification, a letter of attorney is required.
